Our
approach
JI assesses a young person to determine the services, and thus partners, needed. These partners wraparound a young person (age 10-17) and their family to develop a youth-driven customized plan. A JI Coordinated Care Advocate is assigned to the youth, and they establish a relationship with the young person and family members, while seeing the plan through to completion.
What is the first Step?
The referral process for youth and families begins with submitting a referral form. Youth are referred primarily by the Department of Human Services, school districts, and the 4ᵗʰ Judicial District Juvenile Court. Families may also submit a request.
